Full Job Description

Minimum Qualifications:

  • Must be 18 years of age at the time of hire.
  • Must have a criminal record clearance from DOJ.
  • Valid California Driver's license, safe driving record, and license to drive at least two years.
  • Must have a safe and reliable vehicle to transport clients.
  • Must maintain current and adequate auto insurance.
  • Must obtain CPR/FA certification within 60 days of hire.
  • COVID Vaccine or medical or religious exemption
  • Must be able to work evenings and weekends
  • Some College Units

Primary duties:  As a direct service provider, the client's facilitator will assist the service recipient in developing functional skills through one-to-one training in the community and natural settings, consistent with the service recipients' Individual Service Plan (ISP).  The facilitator will provide service recipients with high-quality community integration training, volunteer opportunities, and employment development and exploration. Conducts client contacts and schedules and weekly activities consistent with the clients' Individual Service Plan (ISP) long and short-term goals. A facilitator will also assist the client and other planning team members with the intake and assessment process if the Program requires it. Depending on the physical needs of clients, some self-care assistance, such as toileting and feeding, may be necessary to support the client's success.
