Case Manager  - Adult Development Center (ADC)

Primary Duties:

Supervises, coordinates, reviews, and maintains consumer case management and record-keeping systems of assigned Programs and caseload. Communicates with employers with sufficient frequency to ensure longevity and security of consumer employment opportunities. Manages and supervises assigned staff (Client Facilitator) in all areas, including interviewing, hiring, training, information sharing, counseling, disciplinary actions, and termination. Ensures the delivery of consumer paychecks each payday, if applicable. Works in close coordination and cooperation with the Program Managers, Director. Communicates effectively and maintains a cooperative working relationship with representatives from regulatory agencies, funding and referral sources, care providers, family members, and all other entities as required. Conducts meetings with assigned staff according to a schedule agreed upon with the supervisor. Coordinates and schedules job coaching services to consumers to demonstrate daily the organization’s Vision, Mission, and Core Values. All other duties as assigned.

Case Manager reports to assistant director.

Case Manager supervises no other staff in the facility.

Minimum qualifications:

  • Must be at least 18 years of age.
  • Must Pass Health Screening and TB test.
  • Must be first Aid and CPR certified.
  • Must pass Criminal Record Clearance (DOJ and FBI Background check)
  • Associate degree in social science or commensurate experience includes three years of experience providing services to people with disabilities and at least one year in staff supervision and program coordination.
  • Must have a valid California Driver’s license, have 24 months of driving experience.
  • Experience in consumer evaluation, record-keeping, or case management, plus a demonstrated ability to work with individuals with developmental disabilities and individuals with disabilities such as mental illness, substance abuse, and physical and medical disabilities preferred.
  • Knowledge of Rehabilitation and San Diego Regional Center referral and funding procedures.
  • Work hours are Monday-Friday, 7:00 am-3:00 pm, or beside on client and company needs.
